Youth Transition Services Specialist
Part-time position with the area's only Independent Living Center, a non-profit providind services for people with disabilities. The Youth Transition Services Specialist will work a member of the cores services team to empower youth and young adults with disabilities for a successful transition to adulthood through classroom activities and one-to-one skill development.
Position Title: Youth Transition Services Specialist
Part-time position (22.5 to 25 hours per week) with the area’s only Independent Living Center, a non-profit providing services for people with disabilities. The Youth Transition Services Specialist will work as a member of the core services team to empower youth and young adults with disabilities, ages 14 through 24 years, for a successful transition to adulthood through classroom activities and one-to-one skill development. Compensation is $20 - $22 per hour. Mileage reimbursement for travel in Southwestern Ohio. No benefits are offered with this position.
Essential Job Functions:
- Facilitate the transition of youth with significant disabilities to postsecondary life, including those who have reached the age of eighteen and are still receiving services in accordance with an Individualized Education Program (IEP) .
- Assist with developing engaging, age-appropriate curriculum for students to prepare them for transition into adulthood.
- Maintain communication with the parent or guardian.
- Facilitate school or community-based, one-on-one instruction for students with disabilities focused on Self-Advocacy and Workplace Readiness Training
- Provide instruction on empowerment, self-advocacy, and disability pride to youth in individual and group settings.
- Assist youth with disabilities in reaching and maintaining their highest level of independence through individualized skills training.
- Mentor youth and provide opportunities for growth and leadership.
- Refer consumer to appropriate community resources when needed.
- Assist with developing individual Independent Living Plans.
- Report on progress toward goals, successes, and program activities.
- Maintain all required documentation.
- Establish and maintain relationships with community partners.
- Perform additional assignments as delegated by supervisor in a timely manner.
- Maintain confidentiality of consumer information.
Position Qualifications:
- BA or BS in Education, Special Education, or Social Services
- Prior experience in instruction and training for youth with disabilities
- Excellent oral and written communication skills
- Able to organize and manage time in order to complete tasks.
- Able to engage youth through positive interactions.
- Proficient with using video conferencing services.
- Prefer strong knowledge of Americans with Disabilities Act (ADA), Individuals with Disabilities Education Act (IDEA), 504 plans, and the Individualized Education Program (IEP), and the transition process for youth.
- Understand the Independent Living Philosophy.
- Valid driver’s license and car insurance with reliable personal transportation required.
