Transitions, Inc. is looking for licensed professionals to join our team with opportunities within Residential, Intensive Outpatient and Partial Hospitalization Services. Position Title: Substance Use Disorder Therapist – Residential (Full-time, Part-time)
Supervisor: Clinical Supervisor
Qualifications: Master’s Degree in Behavioral Health with active Kentucky License including CSW, LCSW, LPCA, LPCC or LPA and/or Certified by the state of Kentucky as a Certified Alcohol and Drug Counselor (CADC).

Job Responsibilities:
- Manage a clinical caseload and provide quality of care services to clients including clinical assessments, individualized treatment plans, individual therapy and group therapy.
- Participate in weekly multidisciplinary team meetings with support staff.
- Assure documentation is in compliance with contract regulatory and agency requirements and best clinical practices and complete all clinical documentation within expected timeframes.
- Participate in weekly individual supervision with your Licensed Clinical Supervisor.
- Represent Transitions, Inc. in a positive and professional manner in all community interactions.
- Provide high quality, compassionate guidance in both individual and group counseling sessions.
- Plan, oversee, facilitate and document a client’s treatment.
- (Residential and PHP Only) Meet weekly with each client on your caseload.
- (IOP Only) Meet with clients individually as needed.
- Facilitate family sessions.
- Create, review, and update individual treatment plans for each client on your caseload.
- Conduct initial assessments to determine the client’s diagnosis and level of care recommendation.
- Evaluate patient needs and determine if referrals to other programs or facilities are needed.
- Create and facilitate a discharge plan for each client on your caseload, including referring clients to a lower level of care and creating a safe housing plan.
- Collaborate with multidisciplinary team to determine clinical recommendations for each client’s length of stay and discharge plans
- Communicate and coordinate with referral sources, as necessary
- (PHP Only) Communicate with each client’s Targeted Case Manager on a weekly basis
- Provide crisis intervention to clients as needed.
- Act as a liaison between referral sources and clients.
- Performs other related duties as assigned.
