The Qualified Intellectual Disability Professional (QIDP) will ensure the design, delivery and coordination of services through person centered planning and active treatment programs for approximately sixteen ICF residents.

General Statement of Duties:
The Qualified Intellectual Disability Professional (QIDP) will ensure the design, delivery and coordination of services through person centered planning and active treatment programs for approximately sixteen ICF residents.

Essential Functions of the Position:
Interacts with residents and staff with high attention to engagement. Must be alert to and interested in the needs of children and adults with significant developmental disabilities that have complex medical and adaptive needs.

Coordinate and successfully implement all aspects of a resident’s development, including but not limited to medical, social, education and spiritual needs and desires.

Collaborates and partners with other departments to accomplish goals. Responsible for residents in two cottages.

Ensure compliance with regulatory agencies. Complete assessments that are required on an annual basis. Ensures thorough assessment of needs, involving clinical staff, including OT, PT, Speech, Medical, Nursing, and others as appropriate.

Develop and maintain a positive relationship with families and guardians to ensure that we are meeting their expectations

Assist in providing resident care, swimming, in-cottage activities, and community outings as needed.
Transports (drives) and accompanies individuals to medical/social service appointments and community integration activities as needed.

Participate in continuing educational programs, meetings and seminars to enhance skills and job knowledge.

Performs other duties as assigned.

Requirements:
Bachelor’s degree in any human  services field. 
At least 1 year of experience working directly with individuals with development disabilities required
Experience in healthcare setting and medical social work  preferred
Developed skills in organization, effective verbal and written communication, interpersonal relations, training, diplomacy and follow-up. Thorough knowledge of the Individual Plan (IP) process. Ability to give clear, concise instructions.  Capable of handling multiple projects and demands.
Current driver’s license with fewer than 6 points required.
Flexibility of schedule is required in order to meet operational needs.
