Job Summary

As part of a mission focused, innovative, clinical program, the Intake Coordinator will work within an interdisciplinary team to provide services to youth involved in the juvenile justice system. This will include working with treatment staff and court personnel to coordinate care for clients in the Lighthouse Individualized Docket Services (LIDS) program. The staff member will work closely with possible clients, Lighthouse staff, court personnel, and other treatment providers to meet client needs and access to services.

Intake Coordinator

Competitive pay based on licensure and education

Starting at $45,000 and up

$2,000 Sign-on Bonus

Function of Position:

  • Meet with probation officers for possible client referrals gaining access to the Lighthouse Individualized Docket Services (LIDS) program
  • Assist the family in navigating the referral process for more efficient and effective engagement on the docket
  • Identify clients that best meet referral criteria and work with families and court personnel so that paperwork is properly completed
  • Work collaboratively with agencies, courtroom personnel, and family members involved with Juvenile Court
  • Assist the court to screen and process clients for programming, including conducting initial assessments (including but not limited to diagnostic assessments and CANS) with clients and family and integrate into the court functions as much as possible
  • Attend various court meetings including plea hearing, disposition hearings, and others as required
  • Complete follow up with all families who have completed the program
  • Track client referrals through the process until acceptance onto the docket and track appropriate metrics for client referrals as well as data for quality improvement and outcomes
  • Complete and maintain documentation of services
  • Carry a small caseload, as needed

 

Minimum Qualifications:

  • Professional licensure in State of Ohio as a Counselor (LPC), or Social Worker (LSW).
  • Bachelor's Degree in Social Work or related field
  • Court and treatment services experience preferred
  • Understanding of or willingness to learn and apply trauma-informed care principles
  • Valid license, insurance and good driving record required
